What to record

  • When it happened: the date, approximate time, and location.
  • What happened: threats, physical contact, property damage, or coercive behavior.
  • Injuries or damage: bruises, medical care, broken items, or photos you collected.
  • Who saw or heard it: names of witnesses, neighbors, or children present.
  • Reports you made: police report numbers, CPS notices, shelter intakes, or school contacts.
